Zygomycosis: Epidemiology and Treatment Options*
نویسنده
چکیده
Although it has hitherto been considered a rare type of infection, the incidence of zygomycosis (also known as mucormycosis) appears to be increasing. Zygomycosis is almost always lethal if not aggressively treated, and often lethal even with appropriate medical management. Zygomycetes are generally opportunistic organisms, although infection can occur in a normal host. Aerosolization of spores occurs easily, and the portals of entry include the respiratory tract, a disrupted integument, and ingestion of contaminated food. Zygomycetes are often angioinvasive and can manifest as sinusitis, pneumonia, skin infection, and sometimes gastrointestinal infections or necrosis. Patterns of infection vary by the host’s underlying condition. Histopathologically, one sees wide, thin-walled, “ribbony,” aseptate hyphae. Some reports suggest that an increase in zygomycotic infections may be because of increasing voriconazole use. Because Zygomycetes are not susceptible to voriconazole, their emergence in the presence of this drug is not unexpected. The optimal treatment for zygomycosis should include a combination of surgical debridement, medical antifungal therapy, and efforts to improve host defenses. Amphotericin B is the firstline choice for zygomycosis treatment. Posaconazole has shown some efficacy for zygomycosis, but it is not currently approved for its treatment; it is only available for compassionate use. (Adv Stud Med. 2006;6(6C):S526-S530)

We report a diabetic patient who developed skin lesions on her left forearm and ulnar nerve involvement following admission to the hospital and putting IV catheter for her treatment. Biopsy samples wee taken from her skin lesions and ulnar nerve and with further evaluation after 2 months, the diagnosis of fungal infection due to zygomycetes and mucur was considered based on the presence o...
متن کاملPosaconazole is effective as salvage therapy in zygomycosis: a retrospective summary of 91 cases.
To evaluate the activity of posaconazole for treatment of zygomycosis, a disease for which therapeutic options are limited, we conducted a retrospective study including 91 patients with zygomycosis (proven zygomycosis, 69 patients; probable zygomycosis, 22 patients). Patients had infection that was refractory to prior antifungal treatment (n=81) or were intolerant of such treatment (n=10) and p...
متن کاملGlobal epidemiology of cutaneous zygomycosis.
The large majority of cases reported worldwide as zygomycosis are infections caused by fungi belonging to the order Mucorales. These infections are invasive, often lethal, and they primarily affect immunocompromised patients. Cutaneous zygomycosis is the third most common clinical presentation, after sinusitis and pulmonary disease. Most patients with cutaneous zygomycosis have underlying disea...
متن کاملCurrent experience in treating invasive zygomycosis with posaconazole.
The treatment of zygomycosis has two cornerstones, namely, surgery and antifungal drugs. In many patients, both need to be applied to achieve treatment success; without treatment, the mortality rate of zygomycosis approaches 100%. Because treatment options are limited, no well-designed randomized clinical trial has been conducted and data are predominantly derived from compassionate-use program...
متن کاملRhinocerebral zygomycosis with pansinusitis in a 14-year-old girl with type 1 diabetes: a case report and review of the literature
BACKGROUND Zygomycosis is a rare life-threatening fungal infection affecting mostly patients with predisposing conditions such as diabetes mellitus, immunodeficiency, haemochromatosis or major trauma. METHODS We describe a case of rhinocerebral zygomycosis in a girl with type 1 diabetes and review previous published cases and treatment options. RESULTS A 14-year-old girl with type 1 diabete...
